I use a handy little product called MenuEverywhere (http://www.binarybakery.com/menueverywhere.html) that displays the current app's menu in the title bar for the currently active window. This is particularly nice if you like to have a large screen with multiple apps open and don't want to go to the top of the screen to make menu choices.
It doesn't seem to work with MacDive, however. According to the MenuEverywhere FAQ, "MenuEverywhere works very well with most programs. Issues may arise in applications that do not rely on standard windows."
